As a client of the Family Mediation Trust, we require you to sign a few documents that will help us manage your case effectively. These documents include the Agreement to Mediate, and Mediation Voucher Consent form.

Agreement To Mediate

This document outlines the process and rules that are essential for the delivery of mediation meetings.

The agreement in this form will only be used if your case reaches mediation.


Voucher Scheme Client Consent

The document provides the Family Mediation Trust with consent to apply on behalf of the client for the Family Mediation Voucher worth £500 from the government.

THE DOCUMENT CAN BE LOCATED AT THE TOP OF THIS PAGE.

The Family Mediation Trust will only make the application for the voucher in agreement with both clients during the MIAM or mediation sessions.  

If your case DOES NOT include child issues you will not be eligible for the voucher. If your case DOES NOT include child issues please select this option as you will not be eligible for the voucher.
